چکیده

Reconstruction of flexor tendons the hand injuries is performed under general or regional anesthesia and tourniquet, in general. During Covid-19 pandemic period insufficiency anesthesiologists created need to find alternative methods with similar results existing ones. Wide awake, local anaesthetic, no-tourniquet technique (WALANT) started be more popular, based on advantages (low cost, no access anaesthetic support, intraoperative collaboration control active motion tendon reparation) gradually appeared a safe technique. Combination lidocaine adrenaline allows ensure at same time vasoconstriction. The aim this study estimate evidence effectiveness secure WALANT case series from our reconstruction unit according patient’s satisfaction, complications, functional outcomes. Material & Method: April 2021 September 2022, 34 patients traumatic lesion were treated tenorrhaphy procedure. zone injury was I,II,III, T1,T2,T3. All operated within 24 hours able discharged day. majority operations second operative theatre all without support. Results: Overall, Visual Analogue Scores 0/10, mean rate satisfaction very satisfied 82.3%, while Quick DASH score 1.38. There cases peri-postoperative complication during In there administrate phentolamine as antidote vasoconstriction other procedure anesthesia. Conclusions: method which you can pain blood loss combination adrenaline. absence tourniquet avoid intra- postoperative discomfort patient, patient offers immediate restoration complications tenorrhaphy. Furthermore, patients̓ continues exist high nonentity.

Wide awake local anaesthesia no tourniquet technique (WALANT)

Until recently most hand surgeries was performed with a tourniquet to provide better visibility. The discomfort of the tourniquet is very unpleasant and unnecessary for patients. To avoid this, we have traditionally relied on an anesthesiologist to give sedation, brachial plexus or Bier block, or general anesthesia. A good alternative to traditional tourniquet hand surgery is to use only 2 medi...

Wide-awake flexor tendon repair.

The wide-awake approach to hand surgery is performed with no sedation and no tourniquet. Only locally injected tumescent lidocaine and epinephrine are used for anesthesia and hemostasis. The confirmation that epinephrine hemostasis in the finger is safe1–5 has permitted the use of this technique, which has now become widespread in Canada. With exceptions that include hand surgery in small child...

Wide-Awake Primary Flexor Tendon Repair, Tenolysis, and Tendon Transfer

Tendon surgery is unique because it should ensure tendon gliding after surgery. Tendon surgery now can be performed under local anesthesia without tourniquet, by injecting epinephrine mixed with lidocaine, to achieve vasoconstriction in the area of surgery. This method allows the tendon to move actively during surgery to test tendon function intraoperatively and to ensure the tendon is properly...

Wide Awake Flexor Tendon Repair in the Finger

W ide awake flexor tendon repair means no tourniquet and no sedation tendon repair under pure local lidocaine and epinephrine finger and hand anesthesia. The 5 main advantages of doing the repair this way in the unsedated patient are as follows: (1) fewer postoperative ruptures happen because intraoperative testing of the tendon repair reveals gaps in 7% of cases that are repaired before skin c...

Achieving the optimal epinephrine effect in wide awake hand surgery using local anesthesia without a tourniquet.

BACKGROUND In our experience, for all surgeries in the hand, the optimal epinephrine effect from local anesthesia-producing maximal vasoconstriction and visualization-is achieved by waiting significantly longer than the traditionally quoted 7 min from the time of injection. METHODS In this prospective comparative study, healthy patients undergoing unilateral carpal tunnel surgery waited eithe...
